{"id":337,"date":"2014-03-03T10:07:37","date_gmt":"2014-03-03T19:07:37","guid":{"rendered":"http:\/\/www.keenanpowell.com\/blog\/?p=337"},"modified":"2014-03-03T10:07:37","modified_gmt":"2014-03-03T19:07:37","slug":"what-are-these-forms","status":"publish","type":"post","link":"https:\/\/www.keenanpowell.com\/blog\/2014\/03\/03\/what-are-these-forms\/","title":{"rendered":"What are these forms?"},"content":{"rendered":"<p>&nbsp;<\/p>\n<p><a href=\"https:\/\/i0.wp.com\/www.keenanpowell.com\/wp-content\/uploads\/2013\/12\/bigstock-Silhouette-1113353.jpg\"><img data-recalc-dims=\"1\" decoding=\"async\" loading=\"lazy\" class=\"aligncenter size-full wp-image-284\" alt=\"bigstock-Silhouette-1113353\" src=\"https:\/\/i0.wp.com\/www.keenanpowell.com\/wp-content\/uploads\/2013\/12\/bigstock-Silhouette-1113353.jpg?resize=625%2C417\" width=\"625\" height=\"417\" \/><\/a><\/p>\n<p>After you\u2019re injured at work, you can expect to receive a letter from the insurance company with several releases.\u00a0\u00a0 Do you have to sign them?<\/p>\n<p>Maybe.\u00a0It depends on what they are asking for.<\/p>\n<p><span style=\"text-decoration: underline;\">Medical Release<\/span>:\u00a0 The insurance company is entitled to all of the medical records relating to treatment of you work injury.\u00a0 Also, as a general rule, the insurance company is entitled to all of your medical records relating to the body part that was injured from two years prior to the date of injury.\u00a0 There is a form on the Alaska Workers Compensation Board\u2019s website which has been drafted by the Board.\u00a0\u00a0<a href=\"http:\/\/www.keenanpowell.com\/wp-content\/uploads\/2014\/03\/Release-of-Medical-Information.pdf\">Release of Medical Information<\/a><\/p>\n<p>If the insurance company\u2019s form looks like the Board\u2019s form and is limited to the body part injured at work and from two years prior to your date of injury, it\u2019s appropriate.\u00a0 Additionally, certain providers (like Alaska Regional and Providence) insist on their own forms.\u00a0 As long as these forms are limited to the body part injured at work and from two years prior to your date of injury, they are appropriate.<\/p>\n<p><span style=\"text-decoration: underline;\">Employment Records Release<\/span>: The insurance company is entitled to a list of your employers during the preceding ten years and an employment records release allowing them to speak with your prior employers IF you are applying for reemployment benefits.\u00a0 As a general rule, they are not entitled to this information unless you are applying for reemployment benefits.<\/p>\n<p>If you have questions about your workers compensation claim, call my office.\u00a0 I have more than 30 years experience fighting for the rights of injured Alaskans.
